Overview
Australians are living longer. Traditionally, healthcare has been focused on acute illness. However, the number of people living with chronic disease is growing. This is an important intersection as many chronic illnesses begin as an acute alteration or can cause an acute alteration in health. In this unit, you will analyse lifestyle factors such as obesity, insufficient physical activity, tobacco smoking, and alcohol use and their impact on acute and chronic health conditions. Using a systems-based approach and utilising case studies, you will probe into the journey of healthcare consumers as they experience alterations in cardiovascular, respiratory, endocrine, nervous and haematological health through an acute episode and follow along as their illness extends to a chronic condition. You will consider the nursing process required to support people with acute and chronic conditions and potential consumer deterioration. You will learn how to assist people living with a chronic health condition to implement self-management strategies and develop care plans from a person-centred, community, family and healthcare service perspective to minimise exacerbations. You will develop a discharge/education plan utilising aspects of behavioural change, psychosocial support and health literacy aimed at effecting change and supporting a person's self-management of chronic illness taking into consideration the roles of other members of the interdisciplinary care team. The unit is scheduled to align with the content of Pathophysiology and Pharmacology for nurses 1.

Pre-requisite: Nursing Foundations
Important note: Students enrolled in a subsequent unit who failed their pre-requisite unit, should drop the subsequent unit before the census date or within 10 working days of Fail grade notification. Students who do not drop the unit in this timeframe cannot later drop the unit without academic and financial liability. Attendance Requirements
All on-campus students are expected to attend scheduled classes - in some units, these classes are identified as a mandatory (pass/fail) component and attendance is compulsory. International students, on a student visa, must maintain a full time study load and meet both attendance and academic progress requirements in each study period (satisfactory attendance for International students is defined as maintaining at least an 80% attendance record).
Recommended Student Time Commitment
Each 6-credit Postgraduate unit at CQUniversity requires an overall time commitment of an average of 12.5 hours of study per week, making a total of 150 hours for the unit.

Assessment Grading
This is a graded unit: your overall grade will be calculated from the marks or grades for each assessment task, based on the relative weightings shown in the table above. You must obtain an overall mark for the unit of at least 50%, or an overall grade of 'pass' in order to pass the unit. If any 'pass/fail' tasks are shown in the table above they must also be completed successfully ('pass' grade). You must also meet any minimum mark requirements specified for a particular assessment task, as detailed in the 'assessment task' section (note that in some instances, the minimum mark for a task may be greater than 50%). As this unit is offered online, students are asked to prepare their own individual study plan to undertake self-led and self-directed study throughout the term. 1 Case Study
This written case study assessment requires you to demonstrate evidence-informed clinical reasoning and culturally safe practice in managing an acute-to-chronic cardiovascular condition. You will choose one of the two case scenarios provided—either Jack Williams or Aaliyah Johnson—and apply a structured, analytical approach to address the four key focus areas outlined below. Your discussion must be supported by a minimum of fifteen contemporary (≤7 years) peer-reviewed sources, with additional reputable grey literature used where relevant.
Artificial intelligence tools approved at Level 2 (AI planning)—Microsoft Copilot (protected version), Grammarly, and Studiosity—may be used for editing and proofreading only. Any use of generative AI must be acknowledged on the cover page in accordance with the provided declaration statement.

2 Poster Sessions
This assessment requires you to design and present a concise, evidence-informed academic poster and short oral presentation demonstrating your ability to critically evaluate health behaviour interventions and construct a collaborative, person-centred discharge or education plan. The task builds on Learning Outcomes 3 and 4 by linking theory to practice through a visual and spoken format suitable for professional and academic audiences.
