Overview
This unit will provide advanced knowledge and skills in nursing assessment and care planning. You will learn how to apply a clinical reasoning process and an Aboriginal and Torres Strait Islander health assessment model in order to prioritise and perform holistic nursing care for individuals, families and groups. You will critically examine common risk screening tools and clinical pathways used in practice, to organise and perform safe and effective nursing care. You will evaluate early warning systems and health assessment instruments through evidence-based research and by analysing practice and policy documents. You will consider best practice implications for medication administration, wound care and contemporary nursing psychomotor skills. There are online workshops you will be required to attend. There is a compulsory on campus activity you will have to attend. You will also undertake 160 hours of work-integrated learning which you will undertake after week 12. This will allow you to demonstrate responsible nursing practice under the supervision of a Registered Nurse. Work-integrated learning may be delayed due to travel restrictions and local healthcare service policy.

Pre-requisites or Co-requisites
Co-requisites - NURS20161 and NURS20160 Students must be enrolled in CH79 Graduate Certificate in Nursing (Re-Entry)
Important note: Students enrolled in a subsequent unit who failed their pre-requisite unit, should drop the subsequent unit before the census date or within 10 working days of Fail grade notification. Students who do not drop the unit in this timeframe cannot later drop the unit without academic and financial liability. Attendance Requirements
All on-campus students are expected to attend scheduled classes - in some units, these classes are identified as a mandatory (pass/fail) component and attendance is compulsory. International students, on a student visa, must maintain a full time study load and meet both attendance and academic progress requirements in each study period (satisfactory attendance for International students is defined as maintaining at least an 80% attendance record).
Recommended Student Time Commitment
Each 12-credit Postgraduate unit at CQUniversity requires an overall time commitment of an average of 25 hours of study per week, making a total of 300 hours for the unit.

- Access Quizzes 1 and 2 via the Assessment 1 portal on the Moodle site. This is an open-book quiz, i.e., you can refer to your learning materials at any time during your attempt.

- Each question in Quiz 1 and Quiz 2 is worth 1 mark each.
- You must complete both quizzes in one sitting. Aim
This assessment of this assessment is for you to demonstrate your knowledge and skills you have learned or refreshed over the term to the simulated clinical environment.
Instructions
You are required to attend a five (5) day, on-campus residential school in Cairns, 13th October - 17th October. You will need to be successful in Assessment 1 to move forward to residential.
Further information regarding the Cairns campus, including examples of student accommodation options, is available here.
This assessment consists of the following four (4) components:
Attendance
100% attendance is required, meaning you must attend for the full five (5) days. An attendance sheet will be provided for you to sign each day.
Safety in practice assessment
Length: Fifteen (15) minutes plus handover.
Task: Under exam conditions, you must accurately document the assessment data provided, follow policy accordingly, and answer three (3) multiple-choice questions. You will have 15 minutes to complete this assessment component. Once this is completed, you will provide a handover to a team leader based on your documentation and ability to follow policy. Please see the attached rubric for further information.
Due: Day two (2) of residential school
Feedback: Within one (1) day of the attempt
You must pass this assessment component to achieve a passing grade for this unit. If you do not pass this component, you may be offered one (1) opportunity to re-attempt that only applies to this assessment component. A re-attempt is where you are given a second opportunity to demonstrate your achievement of one or more of the unit’s learning outcomes before progressing to new learning or participating in subsequent learning activities. You may be allowed to re-attempt, however, you must:
- have shown a reasonable attempt to complete the initial assessment task
- be granted a re-attempt by your Unit Lead/Coordinator
- undertake the assessment task that you have failed and resubmit the revised work for marking within seven consecutive days

Reflective Practice
Task: During your Residential School, you are required to reflect on your practice daily. However, on day four (4) of Residential School, you will be required to formally reflect upon your practice via the Nursing and Midwifery Board of Australia (NMBA) Registered Nurse Standards of Practice as per the Residential School Observation Tool (please see below).

Residential School Observation Tool
Task: During Residential School, your clinical practice will be assessed over the entire five (5) days under the NMBA Standards for the Registered Nurse via the Residential School Observation Tool (see below).
Feedback: Returned to students on day five (5) of Residential School.
Requirements: Uniform
During Residential School students are required to always present in appropriate CQU uniform, which includes an appropriate name badge displayed on the lapel of the upper garment.
Please review the Uniform requirements document below with the relevant components for Re-entry highlighted in yellow. You can purchase the uniform from the CQU bookshop, together with the ‘Blue Re-Entry Tag’ (which attaches to the arm sleeve of the scrub top - see picture/s) and a compulsory name badge.
All students are advised to purchase at least two (2) shirts and two (2) pairs of pants to facilitate minimum laundering standards during residential school and placement. Due to high demand, all students are encouraged to order these items as early as possible.
Please remember your name badge will typically display only your first name (or preferred name), i.e., the name you would like to be called during residential school and placement. Therefore, the name on this badge is your choice. For example, a fictional student’s full name might be Michelangelo Antonio Russilino, however, in everyday interactions, the student prefers to be called Mick. Therefore, he will order a badge with Mick as the name.
In alignment with occupational health and safety standards in health care settings, clean black/ navy leather lace-up (or equivalent) closed-in shoes with a non-slip sole are also required.

You are required to upload, as one document, the Safety in Practice assessment rubric and the Residential School Observation Tool into ‘Assessment 2’ in Moodle. You will have three (3) working days to complete this.
The teaching team will retain a photocopy of your marking criteria and provide you with the original copy
You are required to upload this to the Assessment 2 submission area on the NURS20162 Moodle site within 72 hours of completing the assessment.

Length: 160 clinical hours (4 weeks), undertaken in one (1) clinical setting
Please note: you are expected to attend placement for 40 hours per week and work any shift the facility allocates. This means that for the duration of your placement, you must be available to complete shifts comprising of mornings, lates, and night duty, across 7 days (including public holidays as applicable).

Aim
The aim of this assessment is for you to demonstrate your application of theory to practice in a real-world clinical setting to successfully embed the learning from this unit into Professional Experience Placement as a safe practising registered nurse.
Learning Outcomes Assessed:
1. Communicate with patients, families and healthcare teams, adhering to legal and professional standards.
2. Evaluate medications used in Australia and their uses in different groups.
3. Prioritise and plan the safe administration of medications to a variety of patients.
4. Apply the Nursing and Midwifery Board of Australia’s Registered Nurse Standards for Practice in the safe and effective care of individuals, families and the community whilst in the industry-based learning environment.
Instructions:
Please see the Clinical Placement (Work Integrated Learning) portal in Moodle for information regarding the mandatory pre-clinical checks required prior to attending clinical placement. Please note that there are strict deadlines regarding when these checks are due, and therefore, all students are encouraged to begin collating this evidence as soon as possible.
1. Attendance & signed letter of introduction
You are required present your signed letter of introduction to an appropriate staff member on the first day of your placement. This staff member must also sign the letter.
At the end of each shift, you are also required to record your clinical hours on the CQU attendance record, which will be provided to you. The preceptor or supervisor you have worked with needs to verify/countersign this attendance every shift.
At the end of your placement, your signed letter of introduction and attendance record must be uploaded in the appropriate section of your summative ANSAT in SONIA (see below).
2. Reflective practice
Whilst you are encouraged to reflect on your practice daily, you must submit formal reflections at the formative and summative assessment points in alignment with the CQUniversity Australian Nursing Standards Assessment Tool (ANSAT) assessment points (please see the below).
Additionally, whilst on placement, you must attend a weekly online Zoom meeting with your Online Clinical Mentor (see Student Clinical Placement Resource Manual for further information).
3. CQUniversity Australian Nursing Standards Assessment Tool (ANSAT)
Throughout the placement, your clinical practice will be assessed under the NMBA Standards for the Registered Nurse (2016) via the CQUniversity ANSAT. Access to this assessment tool is online via the CQU SONIA app.
Assessment via the CQU ANSAT will occur at a formative (mid-way) and summative (final) assessment point.
You will receive an email from the Work Integrated Learning (WIL) team on the Tuesday of week 2 (formative) and week 4 (summative) of your placement. Please open this email and follow the instructions to access your formative and summative reflection points and the CQU ANSAT.
Please see the instructions provided in Moodle for further information about completing your online reflections and the CQU ANSAT.
Feedback: You will receive feedback on the outcome of your Assessments within seven (7) working days.
Instructions to complete ANSAT
To complete this assessment, you are required to:
1. Complete the Facilitator/preceptor notification form and submit on SONIA on day one (1) of placement.
2. To be eligible for a Pass grade for this unit, you must complete 160 hours of Professional Experience Placement and achieve a score of 3 or higher on each criterion on the summative ANSAT assessment.
3. The ANSAT will be provided to you electronically through the SONIA system, which is located under the ‘forms’ section. You will receive an email via the WIL team with a link to the ANSAT form. Examples of the Formative and Summative Assessments are available on the WIL Moodle site.
4. Complete a Formative ANSAT at the end of 80 hours, or midway through your placement. This requires you to undertake the following steps:
a. Complete the Student formative self-assessment form in SONIA and submit to your assessor when nearing the midway point of placement.
b. Engage in discussion with your assessor after they have completed their feedback in the Formative Assessment in SONIA.
c. The Unit Coordinator will review your ANSAT assessment, add comments and sign it. If you have a score below 3, a learning support plan may be implemented to provide you with clear performance expectations and goals.
d. Review the Unit Coordinator’s comments and complete the Student Declaration and signature section at the end of your formative ANSAT. You will receive an email notification when this is available (Completing these steps will trigger the release of the summative ANSAT).
5. Complete a Summative ANSAT near the end of your placement:
a. Complete the Student summative self-assessment form in SONIA and submit to your assessor near the completion (last day) of your placement.
b. Your assessor will complete your Summative Assessment in SONIA and discuss your assessment and feedback with you. You will be notified by email to complete the Student declaration and signature section on your Summative ANSAT.
c. A score of 3 or higher for each ANSAT criterion on the summative ANSAT assessment is required to pass the summative assessment. If you do not meet each ANSAT criteria at a 3 or above on your summative assessment, you may receive a Fail grade for the Unit.
6. The Unit Coordinator will review your summative assessment, check the hours recorded on your Record of Attendance, and assign a grade.
7. Maintain and record your clinical hours at the end of each shift on your CQUniversity Record of Attendance (available in Moodle under Assessment 2). This record must be verified/co-signed each shift by the RN, preceptor or clinical facilitator you have worked with. The Record of Attendance will need to be uploaded to the NURS20162 Moodle site at the completion of the last day of placement.
Requirements
- Attendance of clinical placement is mandatory for this unit.
- Zoom access for weekly Online Clinical Mentor meetings
- The required hours of clinical practice (160 hours) must be completed in entirety and with a satisfactory ANSAT summative to be eligible for a Pass grade for this Unit. Please see the CQUniversity Clinical Placement Attendance Policy for further information. If you are not meeting satisfactory practice standards at any point of the placement, a meeting will be scheduled with the facilitator, yourself, and the Unit Coordinator to discuss your progress. A support plan may be implemented to assist you to meet the ANSAT criteria satisfactorily. If you do not meet each ANSAT criteria at a satisfactory level (3 or higher) for the summative assessment, you may receive a Fail grade for this assessment.
- Any unsatisfactory performance that jeopardises the safety of people in your care or which does not adhere to the NMBA Registered Nurse Standards for Practice, Code of Professional Conduct, Code of Ethics, and/or Social Media Policy criteria may result in you being removed from placement. Please refer to the Work-integrated Learning-Student Placement Policy and Procedure that can be found on the WIL Moodle site.
- As a nursing student representing CQUniversity, you are required to comply with CQUniversity student nurse uniform requirements and the CQUniversity Student Conduct Policy and Procedure during your professional experience placement. Please refer to the Work-integrated Learning-Student Placement Policy and Procedure that can be found on the WIL Moodle site, and information provided in Assessment 2 – Residential School.
Assessment Criteria
- Students are assessed against the Australian Nursing Standards Assessment Tool (ANSAT). A copy of this is available to you through SONIA.
- Completion of 160 hours of Professional Experience Placement and achieve a score of 3 or higher on each criterion on the summative ANSAT assessment.
- You are required to attempt and submit all assessment items in this unit.
Submission
Submit a copy of your Record of Attendance to the submission area on the NURS20162 Moodle site at the completion of the last day of placement. If you have used an online timesheet in SONIA, please print this when you have completed your placement. Your finalised Record of Attendance must be submitted within three (3) days of completing your placement. Your grade will not be finalised until this step has been completed.
Reflections and CQU ANSAT completed online via SONIA. Formative ANSAT assessment is due at 80 hours of clinical placement; summative ANSAT assessment, signed letter of introduction and attendance record is due at 160 hours of clinical placement.
Outcome will be available within 10 working days from submission of appropriate documents into SONIA
