Overview
In this unit, you will examine chronic health conditions that may affect people across the lifespan. You will learn to recognise symptoms that commonly lead to an exacerbation or worsening of a chronic health condition. Taking into consideration individual socio-cultural and lifestyle choices, you will assist people living with a chronic health condition to implement self-management strategies and develop care plans from a person-centred, community, family and health care service perspective to minimise exacerbations. You will develop a discharge/education plan utilising aspects of behavioural change, psychosocial support and health literacy aimed at effecting change and supporting a person's self-management of chronic illness taking into consideration the roles of other members of the interdisciplinary care team.

Pre-requisites or Co-requisites
Pre-requisites: NURS12161 Professional Experience Placement 2 or NURS12156 Clinical Practice 2BIOH12011 Pathophysiology and Pharmacology 1 or BIOH11006 Advanced Anatomy and Physiology and NURS12154 Pharmacology for Nursing Practice.
Important note: Students enrolled in a subsequent unit who failed their pre-requisite unit, should drop the subsequent unit before the census date or within 10 working days of Fail grade notification. Students who do not drop the unit in this timeframe cannot later drop the unit without academic and financial liability. Attendance Requirements
All on-campus students are expected to attend scheduled classes - in some units, these classes are identified as a mandatory (pass/fail) component and attendance is compulsory. International students, on a student visa, must maintain a full time study load and meet both attendance and academic progress requirements in each study period (satisfactory attendance for International students is defined as maintaining at least an 80% attendance record).
Recommended Student Time Commitment
Each 6-credit Undergraduate unit at CQUniversity requires an overall time commitment of an average of 12.5 hours of study per week, making a total of 150 hours for the unit.

Assessment Grading
This is a graded unit: your overall grade will be calculated from the marks or grades for each assessment task, based on the relative weightings shown in the table above. You must obtain an overall mark for the unit of at least 50%, or an overall grade of 'pass' in order to pass the unit. If any 'pass/fail' tasks are shown in the table above they must also be completed successfully ('pass' grade). You must also meet any minimum mark requirements specified for a particular assessment task, as detailed in the 'assessment task' section (note that in some instances, the minimum mark for a task may be greater than 50%). This written assessment comprises four equally weighted content-based questions and two additional criteria focused on the appropriate use of academic referencing. The assessment is designed to evaluate students’ understanding of culturally responsive communication, chronic disease management, and nursing care for Aboriginal and Torres Strait Islander Peoples. Each section is marked according to the depth of analysis, critical thinking, and integration of evidence-based practice.
Question 1: Socio-cultural Factors and Text-Message Interventions (20%)
This criterion assesses the student's ability to identify and analyse the socio-cultural factors influencing Aboriginal and Torres Strait Islander Peoples' health, particularly in relation to digital health interventions such as text messaging. Marks will be awarded for insight into cultural considerations, digital access, and health literacy.
Question 2: Challenges and Nursing Approaches (20%)
This section evaluates the student's understanding of the barriers faced by Aboriginal and Torres Strait Islander clients in managing chronic health conditions, and the culturally safe nursing approaches that can support improved health outcomes. the student's ability is to apply critical reflection and culturally appropriate strategies is essential.
Question 3: Overcoming Barriers and Communication Strategies (20%)
Here, the students are expected to explore common communication barriers and outline effective, culturally respectful strategies nurses can use to promote engagement with care. Emphasis should be placed on collaboration, trust-building, and person-centred care principles.
Question 4: Education Plan for Chronic Conditions (20%)
This question assesses the student's ability to develop a basic education plan that reflects the principles of culturally safe care. The plan should demonstrate how nurses can support Aboriginal and Torres Strait Islander consumers to manage chronic conditions with a focus on empowerment, self-management, and community engagement.
In-text Citations (5%)
Marks are awarded for appropriate and consistent use of in-text citations in APA 7th style. The sources should support key arguments and demonstrate integration of current, credible, and relevant academic literature.
Reference List (5%)
This criterion evaluates the accuracy and formatting of the student's reference list according to APA 7th style. Marks are awarded for correct citation of at least eight high-quality academic sources, including peer-reviewed journal articles and reputable texts.
Total: 100%

This written assessment requires you to demonstrate your understanding of chronic health management in a community context. Using the case study of Denis Lane (Appendix 1), you will explore the lived experience of someone managing a chronic health condition and apply nursing principles to support community-based care, self-management, and quality of life.

Introduce the purpose and focus of the essay (~100 words).
Define the chronic health condition presented in the case study and explain why it is listed as a National Health Priority Area (NHPA).
Describe the impact of the condition on Denis’s daily life.
Discuss expectations Denis may have of community-based healthcare professionals, supported by evidence-based literature.

This assessment is divided into two parts and is based on a case study of Denis Lane, a person living in the community with a chronic health condition. The student is required to demonstrate their understanding of chronic disease management and community-based care by discussing Denis’s lived experience and developing an appropriate discharge plan. Marks will be allocated across the following key criteria:
Introduction and Conclusion (5%)
This criterion assesses the student's ability to clearly introduce the aim and structure of their paper and to provide a concise conclusion that summarises the key points discussed. Marks are awarded for clarity, relevance, and alignment with the assessment task.
Lived Experience of Denis (50%)
This is a weighted criterion and evaluates the student's discussion of Denis’s experience living with a chronic condition in a community setting. the students are expected to explore the personal, clinical, and social impacts of the condition, supported by evidence-based literature. Key areas include the relevance of the condition as a National Health Priority Area (NHPA), Denis’s expectations of healthcare professionals, the importance of shared decision-making, and how registered nurses can support culturally safe and person-centred care.
Discharge Plan (30%)
This criterion focuses on the student's ability to create a realistic and culturally appropriate discharge plan for Denis. The plan should address socio-cultural and lifestyle factors, management strategies for exacerbations of coronary artery disease, and the involvement of interdisciplinary team members in ongoing community care.
Professional Writing and Presentation (5%)
Marks are awarded for clear, structured, and professional academic writing. This includes grammar, spelling, formatting, correct use of headings, paragraphing, and adherence to presentation requirements (e.g., font size, spacing, page numbers).
In-text Citations (5%)
This criterion assesses the student's ability to correctly cite supporting evidence within the body of thier text using APA 7th referencing style. In-text citations should be relevant, accurate, and consistently applied.
Reference List (5%)
Marks are awarded for the correct formatting and presentation of the reference list according to APA 7th style. A minimum of eight credible, contemporary academic sources must be used to support the discussion.
